Setting up online forms and the member profile

Every moment somebody signs up with your club goes through a form: online registration, the webshop, the booking wizard, the app, a child registration. This guide explains which forms exist, where to edit them, and how to decide what appears on the member profile.


Finding your online forms

Steps

  1. Go to Settings.
  2. Click Members in the left-hand column.
  3. Choose Forms.

You will see two tabs: Registration and Other. Under Registration you change the layout of registration pages, general conditions and other online forms.


Which forms exist?

Each form has a type and an action. The type decides where the form appears; the action decides what happens with the data entered.

  • Registration — the standard online sign-up. Action: create a member.
  • Inscription — a second registration form, for example in another language or for a separate campaign.
  • WebshopWizard — what the member fills in during a webshop purchase.
  • Coupon — the form used when redeeming a gift voucher or coupon.
  • BookingWizard — what a new member fills in when booking an activity directly.
  • App — registration through the MyClubPlanner app.
  • Leads — this form does not create a member but a prospect. Use it for "request information" pages.
  • ChildRegistration — registering a child under a parent account.
  • Reservation — the details requested when making a reservation.

At the bottom of the list, Add new registration creates an extra form.

💡 Tip: if you are changing a field because members are not completing something at sign-up, check you have the right form. A member registering through the app goes through App, not Registration — that is the classic mix-up.


Deciding what appears on the member profile

Steps

  1. Go to Settings → Members.
  2. Choose Profile.

Modules

  • Under Modules you select which tabs are shown on the member profile: Communication, Sale, Suspensions, Point system, Club balance, Invoices and Fit test. Switch off what your club does not use — it saves your front-desk staff a lot of hunting.

Gender

  • Choose which options are available (Male, Female, Other, Unknown), which is the default, and whether members may change it themselves.

Profile image

  • Decide whether members may add or change a profile picture.

National ID number

  • Choose whether the national ID number is Invisible, Read-only or Editable.

⚠️ Important: the national ID number is sensitive personal data. Only set it to Editable when you genuinely need it, for example for tax certificates, and limit who can see it through authorization groups.

Terms and conditions

  • Enter a URL and a date. Everyone who has not agreed to your terms after that date must agree first when logging into the app or the web platform. This is how you re-collect consent in a controlled way after a change.

Result

You know which form belongs to which sign-up moment, and your member profile shows only what your club actually needs.
